Introduction. IgM monoclonal gammopathy of undetermined significance (MGUS) is defined by asymptomatic circulating IgM monoclonal (M) protein below 30 g/L with a lymphoplasmacytic bone marrow infiltration of less than 10%. 1 IgM MGUS is a pre-malignant condition for non-Hodgkin lymphomas, mostly Waldenström macroglobulinemia (WM), chronic lymphocytic lymphoma, and plasma cell neoplasms.. Monoclonal gammopathies are conditions in which abnormal proteins are found in the blood. The most common condition linked with these abnormal proteins is MGUS. MGUS causes no symptoms. Diagnosis is often done with a lab test called electrophoresis. Because MGUS is not harmful, it doesn’t need to be treated.

This month, Robert A. Kyle, MD, answers a question about the “faint monoclonal band” of IgG kappa or IgG lambda in a dense polyclonal background. CLINICAL DILEMMA What is your approach to a patient who is referred for an immunofixation showing a “faint band” of monoclonal IgG kappa or IgG lambda in a dense polyclonal background with or without.. In the third scenario, the serum protein electrophoresis with immunofixation detects an IgM-kappa monoclonal paraprotein. The serum IgM level is increased at 732 mg/dL. The patient undergoes a bone marrow aspiration and biopsy that reveals 10% involvement by CD5- and CD10-negative, CD20- and CD38-positive kappa-restricted lymphoplasmacytoid cells.
